- If you have Health Card Number, Please Register Online and login to our Patient services,
which includes: Request for Medical Fitness, Health Card Renewal, Setting upcoming
appointments and Health Card Expiry reminders and online change of address.
- If you dont have Health Card No. for Registration, Create Health Card Number.
- If you are having difficulties registering online, then please fill-in and print the following
Registration Form and visit the nearest DOHMS Health Center for registration.
- If you desire to register later on, please use our Express HC Renewal option to
renew your Health Card.
